CBD and Chronic Pain: Why Isn't It Discussed More Often?
What the research says about CBD, opioids, the endocannabinoid system, and taking a whole-person approach to pain

After more than 30 years as a nurse…
One of the hardest conversations I've had with people living with chronic pain usually begins the same way: "I just want my life back."
Rarely does someone tell me they simply want a lower pain score. What they really want is to sleep through the night. Walk their dog again. Enjoy time with family. Play with their children. Work in the garden. Travel. Cook dinner without needing to sit down every few minutes. Feel like themselves again.
Pain isn't simply about hurting less. It's about reclaiming the parts of life that chronic pain has slowly taken away.

If you've ever asked yourself those questions, you're not alone. More importantly, you're not imagining it. Chronic pain affects much more than the area that hurts. It becomes a whole-body experience.
Chronic pain doesn't stay in one place
Pain may begin in your knee. Your back. Your neck. Or your nerves. But over time it often reaches much farther. It can influence your sleep, your energy, your stress, your mood, your appetite, your movement, and your ability to do the ordinary things that make a day feel like yours.
That's why treating pain as a single, isolated symptom so often disappoints. The nervous system, the immune system, and the stress response are all part of the same conversation — which is exactly what I explore in how chronic stress affects your nervous system and inflammation.
The endocannabinoid system: one of the body's conductors
One reason researchers continue studying CBD is because of a remarkable communication network called the endocannabinoid system (ECS). Although many people have never heard of it, the ECS helps maintain balance throughout the body.
Think of it as one of the conductors of an orchestra. The conductor doesn't play every instrument. Instead, the conductor helps every section work together. The ECS helps coordinate communication involved in pain perception, inflammation, sleep, stress responses, mood, immune function, appetite, and energy balance.
Your body naturally produces signaling molecules called endocannabinoids — most notably anandamide and 2-AG — which interact with CB1 and CB2 receptors throughout the body. CB1 receptors are found primarily in the brain and nervous system and help regulate pain signaling, movement, memory, mood, and appetite. CB2 receptors are found primarily on immune cells and throughout peripheral tissues, where they help regulate immune activity and inflammatory responses.
CBD works differently than THC. Rather than strongly binding directly to CB1 or CB2 receptors, research suggests CBD influences the endocannabinoid system indirectly while also interacting with several other biological pathways involved in pain, inflammation, and nervous-system regulation.
Why does that matter? Because chronic pain affects many of those same systems. That is why researchers continue studying CBD — not as a miracle cure — but as one possible layer within comprehensive pain management.
CBD deserves attention in chronic pain because it interacts with a regulatory system involved in many of the same processes affected by chronic pain.
How different pain medicines work
Every pain medicine works through a different pathway. Understanding those pathways helps explain why one option helps a person enormously and barely touches someone else.
- Opioids act on opioid receptors in the brain, spinal cord, and gut to reduce the perception of pain. They can be appropriate and even necessary in certain situations, but they also carry risks — tolerance, dependence, constipation, sedation, and respiratory depression — which is why they are managed carefully by the prescriber who knows your history.
- NSAIDs (ibuprofen, naproxen) reduce inflammatory signaling through the COX enzymes. They can be helpful for inflammatory pain, but long-term use is associated with stomach, kidney, and cardiovascular concerns.
- Acetaminophen works mainly in the central nervous system and does little for inflammation. Dose limits matter because of liver safety.
- Gabapentinoids (gabapentin, pregabalin) calm overactive nerve signaling and are often used for neuropathic pain. Sedation, dizziness, and swelling are common considerations.
- CBD does not act on opioid receptors and does not work like an NSAID. Research suggests it interacts with the endocannabinoid system and other pain- and inflammation-related pathways, which is why it is studied as a possible supportive layer — not a replacement for prescribed treatment.
Because these pathways differ, they can affect each other. CBD may interact with certain medications through liver enzymes, so any addition should be discussed with your healthcare provider or pharmacist. Pain management rarely succeeds by replacing one thing with another. It succeeds by building layers of support.
- Better sleep
- Gentle movement
- Lower stress
- Improved nutrition
- Managing inflammation
- Physical therapy
- Appropriate medications
- CBD, when appropriate

From my nursing experience
One thing I've learned after more than 30 years of nursing is that people living with chronic pain are often far stronger than they realize.
Many people living with chronic pain have spent years adapting — sleeping less, moving less, giving up activities they love, and quietly accepting limitations because they believe there are no other options.
Sometimes the first step isn't finding the perfect treatment. Sometimes it's understanding that there may be more than one path toward feeling better. That is why I believe education matters. When people understand how their body works, they are better prepared to make thoughtful, evidence-based decisions with their healthcare team.
My hope for you
My goal isn't to convince you to use CBD. My goal is to help you better understand your body so you can make thoughtful, evidence-based decisions with your healthcare team.
You are not broken, and you are not alone. Your body may be asking for support from more than one direction. Healing happens gently, in layers, one small step at a time.
I hope you give yourself grace. I hope you trust the small changes. I hope you keep reaching for the pieces of life that bring you comfort, strength, and peace. Because the goal is not perfection. The goal is reclaiming the parts of life that pain, illness, inflammation, and limitations may have tried to take away.
